Integrated Care Coordination

EPH Mental Health reviews mental health concerns, current services, medical needs, treatment barriers, support systems, and personal goals. 

EPH Mental Health develops a coordinated plan with appropriate services, referrals, providers, resources, and treatment goals. 

Care coordination connects individuals with therapists, psychiatric providers, medical professionals, community programs, and additional support services. 

With appropriate consent and authorization, authorized healthcare professionals may share relevant information to promote consistent, organized care. 

Regular follow-up reviews service access, referral outcomes, treatment progress, changing needs, and additional support requirements. 

The process begins with an assessment of mental health needs, current services, medical concerns, support systems, treatment barriers, and personal goals. 

EPH Mental Health identifies appropriate providers, programs, referrals, and community resources based on documented care needs. 

Care coordination may include referral follow-up, appointment preparation, scheduling help, transportation resource information, and communication about accessing services. 

With appropriate authorization and consent, the healthcare team may share relevant information to support organized, consistent care.  

Ongoing follow-up helps identify changing needs, referral outcomes, treatment barriers, and additional support requirements. 

Case management support may assist during provider changes, program changes, hospital discharge, higher levels of care, medication service changes, and movement between behavioral health services.  

Integrated Care Coordination may include assessment, care planning, referrals, appointment support, provider communication, resource navigation, transition planning, and follow-up. 

Integrated Care Coordination is not a replacement for therapy. Case management integrates with therapy to help clients access services, make referrals, connect to resources, communicate, organize treatment, and receive practical support. 

 With appropriate authorization and consent, EPH Mental Health may communicate with relevant medical and behavioral health providers. 

Referral support may include identifying appropriate services, providing referral information, coordinating appointments, following up on access to services, and addressing treatment barriers. 

Enrollment begins through the EPH Mental Health intake team by phone, email, or online appointment request.
