✔ Confirm Your Appointment

Check your appointment date and time in your confirmation email or text message. If you need to reschedule, please notify us at least 24 hours in advance. 

✔  Bring Important Forms

Please bring the following to your visit:

• Photo ID
• Insurance card (if applicable)
• List of current medications (including doses)
• Any recent lab results or medical records
• List of questions or health goals

✔  Review Your Health History

Before your visit, think about:

• Any changes in your health since your last visit
• Recent symptoms or concerns
• Family medical history updates
• Lifestyle changes (sleep, stress, diet, activity)

✔  Prepare for the Visit

If You Are Coming In- Person

• Arrive 10–15 minutes early
• Wear comfortable clothing
• Plan for parking and check-in time

If You are Using Telehealth

• Test your device (phone/tablet/laptop)
• Make sure your camera and microphone work
• Choose a quiet, private space
• Join the call 5–10 minutes early
